Parent/Guardian - consent to participate

The volunteers of Scarborough Chapter of Professional Engineers Ontario have organized a Bridge Building Competition for the benefit of school children in our community. A parent or legal guardian who wants their child to participate in this competition must complete and sign this form. Each child must present this completed and signed form at registration on before the competition starts.

I understand that the Scarborough Chapter of Professional Engineers Ontario (SCPEO) and its representatives will take all reasonable steps to provide an environment in which everyone’s safety is of prime importance. SCPEO volunteers have arranged the competition, but SCPEO cannot take direct care and control of your child. We encourage you to be present at the competition so that both of you will share and enjoy your child’s successes and achievements.

I am aware that SCPEO and its volunteers cannot assume responsibility for any injury, loss, damage or harm to any child or to his/her property during the course of any activity, which is part of the above program, including travelling to and from the competition site. I understand and acknowledge that certain risks of injury, loss, damage or harm are inherent to participation in any program or activity and I agree to indemnify and save harmless Professional Engineers Ontario, National Engineering Week, Scarborough Chapter of Professional Engineers, prize sponsors, officers, employees, agents and volunteers from and against any injury, loss, damage or harm that may befall my child(ren) as a result of their participation in this competition.

Personal information collected will be used to conduct this competition and recognize participants and winners. Names and photographs and articles written on this event will be posted on SCPEO’s Internet web site and be used to promote this and future events e.g. solicit individuals and companies for financial sponsorship and/or prizes. Permission is hereby given for any photographs, videos or other recordings made before, during or after this event of my child and/or their bridges to be used as described above.

I hereby grant permission for my child to participate fully in this Bridge Building Competition. I declare having read and understood the above and hereby consent to my child participating on the basis described.

Parent or Teacher - bridge test qualification

I confirm that I have seen this bridge tested and that it can carry at least a 4kg weight suspended from its centre.
